Loopback Analytics Partners with Brookdale Senior Living for Implementation of CMS’s Health Innovations Challenge Grant

Brookdale Senior Living (BSL) through its partnership with the University of North Texas Health Science Center (UNTHSC) and Florida Atlantic University (FAU) has received a significant portion of the Centers for Medicare and Medicaid Services Health Innovations Challenge grant totaling $7.3 million. Loopback Analytics will partner with Brookdale, UNTHSC and FAU to provide a comprehensive care transition management technology platform.

Loopback Analytics’ cloud-based platform integrates data across a myriad of health care information systems, empowering providers to identify at-risk patient populations, match them with appropriate interventions and evaluate results to ensure consistent quality care. With multimodal communications, the solution allows patients and caregivers to stay connected after hospital discharge and throughout the care continuum. Dashboard reporting provides real-time analysis to measure clinical and economic efficacy of various care interventions.

The purpose of the grant is to implement and clinically evaluate INTERACT (Interventions to Reduce Acute Care Transfers) within 67 BSL facilities. This will involve BSL’s Care Transitions Program which engages residents in nursing homes, assisted living as well as home care settings. INTERACT is an evidence-based quality assurance program designed to reduce avoidable readmissions for residents in long term care. INTERACT was created by a FAU team led by Joseph Ouslander, M.D., Professor of Clinical Biomedical Science and Senior Associate Dean for Geriatric Programs at FAU. The program’s goal is to reduce the number of avoidable and expensive hospital admissions while maintaining a high level of clinical care. Expected savings over the three-year grant period are estimated at more than $9 million.

“The sooner a health issue can be identified, the sooner the intervention can be initiated, which improves the odds of a positive health outcome for our residents,” said Brookdale’s Chief Medical Officer Kevin O’Neil, M.D. “The key to quality in health care is consistency, and this tool has been instrumental in giving doctors a quality assessment of a resident’s health, preventing unnecessary hospitalizations.”

Brookdale Senior Living is the largest senior housing provider in the United States with 646 retirement communities in 35 states with 67,000 residents. A total of 27 communities in Texas and Florida will be the initial focus for the grant, with plans to expand rapidly to other communities throughout the United States.
